Medical License Information

Medical License: Please provide information for one state where you hold a current, valid, and unrestricted medical license. If you only hold a medical license outside of the U.S., please provide a copy of your current, unrestricted medical license from a valid non-U.S. licensing body. You may also need to submit an official verification or letter of good standing from the non-U.S. licensing body, as necessary.

General Certificate(s) and Specialties

Instruction: What board(s) associated with ABMS certification(s) do you hold, or have you previously held, that you wish to certify in with NBPAS? Click on more than one specialty and/or subspecialties as appropriate.

Note: If you select a certificate for a procedural specialty (i.e. a surgical specialty, interventional cardiology, etc.) you will be asked to supply information about the hospital or outpatient facility in which you currently hold privileges in that specialty.

Never ABMS Certified: If you have never been certified by an ABMS member board, NBPAS will not be able to certify you. We realize there are several other very credible national and international medical boards. Unfortunately, we can only certify ABMS diplomats. There are no exceptions.

Appeals: NBPAS decisions can be appealed by writing to info@NBPAS.org and asking for an appeal by the Appeals Committee.

I hereby consent to the disclosure, inspection and copying of information and documents relating to my credentials and qualifications and performance (“credentialing information”) by and between the NBPAS and other healthcare organizations with which I have been affiliated.  I authorize the NBPAS and its/their representatives to consult with representatives of other healthcare organizations with which I have been affiliated.
