The Gatekeeper Driving Doctors From Medicine

For years, physicians have expressed concern about the increasing burden and uncertain value of mandated “maintenance of certification”(MOC) programs. Now, that issue is gaining national attention. In The Wall Street Journal, NBPAS President Dr. Paul Teirstein highlights how the American Board of Internal Medicine (ABIM) and the American Board of Medical Specialties (ABMS) have built a runaway monopoly that holds physicians’ careers and livelihoods hostage.
The op-ed captures what many physicians experience every day: costly, time-consuming testing requirements that add administrative pressure but no measurable benefit to patient care. Dr. Teirstein points out that despite decades of MOC programs, there is no evidence they improve outcomes, but mounting evidence that they contribute to burnout and early retirement.
This piece finally brings national attention to a long-standing frustration among physicians and highlights the urgent need for a modern, evidence-based certification system that promotes clinical excellence over bureaucracy.

Rethinking Physician Certification: A Call for a Modern, Meaningful Standard

“The ABIM invested heavily in psychometrics, but the link between those tests and real-world patient outcomes was tenuous at best.”
In this thought-provoking editorial, former ABIM board member, Dr. Harlan Krumholz, calls for a re-examination of physician certification. He argues that despite decades of MOC programs, there is no evidence they have meaningfully improved patient care, while evidence shows they contribute to burnout and early retirement, outcomes that clearly harm patients.
Krumholz urges a shift toward modern, evidence-based pathways for lifelong learning that measure real-world competence rather than high-stakes, multiple-choice exams. His message reinforces what NBPAS has advocated from the start: certification should evolve to support physicians in practice, not burden them.

Meet our October Champion of Change: Monica Berner, MD
This month, we celebrate Monica Berner, MD, for her outstanding leadership in bringing NBPAS acceptance to Health Care Services Corporation (HCSC)—the largest customer-owned health insurer in the United States, providing access to nationwide care through plans in Illinois, Montana, New Mexico, Oklahoma, and Texas.
At HCSC, Dr. Berner currently serves as the Senior Vice President of HCSC Markets, where she leads growth initiatives across group, individual, and family markets—drawing on her extensive background in Family Medicine and patient care. Previously, she served as HCSC's Chief Clinical Officer, overseeing clinical policy and operations designed to improve access and support the efficient delivery of high-quality, affordable medical care across networks.
In announcing the addition of NBPAS, Dr. Berner shared:
“This was an easy decision. As an NBPAS-certified diplomate, I know that all NBPAS-certified physicians have been board certified in their specialties or subspecialties and are committed to lifelong learning.
